Patient requirements
Our program is designed for patients who are:
- 21 years or older
- Medicaid-eligible
- Diagnosed with a substance use disorder
- Recommended for 3.3 ASAM level of care
Our program combines substance use disorder treatment and co-occurring mental health support.
Notes and exclusions
We may need additional documentation:
In certain situations, we request documentation from probation officers, mental health counselors, or case workers. We’ll work together if that’s the case.
At this time, we are not able to admit patients with:
- Severe or persistent mental illness
- Significant physical health conditions
Insurance coverage
We’ll assist with obtaining health plan authorizations and helping uninsured patients apply for Medicaid.

Current copies of a patient’s drug and alcohol assessment can be faxed to 206-589-6601 or submitted by email.
Before starting treatment, read the frequently asked questions and review the packing list. Our Admissions Team will assist with applying for Medicaid and getting health plan authorization.
